The plan provides broad cover for the medical treatment of illnesses and accidents requiring in-patient hospitalization and also features an optional critical illness cover.

Plan Options

The Healthy Living Group Insurance Plan (HLGIP) is available in 3 options: Individual, Couple and Family. Key details to note:

  1. HLGIP will offer coverage to persons from birth onwards, with a maximum age of entry being 59 years 

  2. An individual and/or their family members; namely spouse and dependent children, are eligible to buy this cover 

  3. Your premium at renewal may change due to a change in your age or changes in the applicable rate 

  4. A maximum of 10 members can be added to a single policy. For example, a policy can consist of two (2) parents and eight (8) dependents.

Eligibility

Persons applying for the HLGIP must be a member of the AffinityPlus Credit Union. To apply, members can submit the required HLGIP Application Form, as well as the Group Health Statement for themselves and their dependents.

How Do I Enroll?

  1. Fill out the Application Form and Health Statement (available from any of our branches), outlining your personal information and health profile. Ensure that the information given on the form is complete and accurate.

  2. Submit the Application Form and premium amount to your Credit Union representative.

Payment Options

  1. You may choose to make one or multiple payments monthly

  2. By Payroll Deduction (recommended to ensure uninterrupted coverage)

  3. Over the counter payments at any branch
